A:	Copies of state licenses and DEA certificates for all participating dispensers A list of desired medications A contact person/phone number at your practice




Q:	Who is a prescriber?
A:	A prescriber is defined as anyone that holds a current DEA license in a particular state. This can include physicians, physician assistants (PA), certain registered nurses and veterinarians.
